Submission of Employee's Medical Records:

An injured employee is required to furnish and provide medical records pertaining to the work injury when requested or required by the Commission. Upon filing a claim with the Commission, the employee also grants the Commission and its office authorization to acquire or subpoena his or her medical records with any medical provider.

 

Any medical record or evaluation as required of the employee to provide and furnish before the Commission concerning the employee's work injury, the progress of on-going medical treatments or rehabilitation of the current injury, or any information pertaining to other medical services required to treat such injury should be conducted by the employee's primary treating physician or a certified medical doctor.

The submission of medical records to the Commission's Office via online is restricted. If such submission is warranted, the medical provider must ensure such records are encrypted with a password, or treated in a way only the Commission's Office can access. This is solely to protect and safeguard the confidentiality and privacy of the employee's medical records.
