Issuance of Notice of Workmen's Compensation Coverage Secured:

Any time a carrier successfully purchases or renews an insurance policy, the carrier is mandated to issue the employer a notice within 10 days to verify a policy coverage has been purchased and secured. This coverage notice is entitled as Notice of Workmen's Compensation Coverage Secured.

Obligation to Provide Coverage Notice to the Commission:

Carriers are obligated to inform and instruct the employer to provide a copy of its Notice of Workmen's Compensation Coverage Secured and to file a Certificate of Compliance with the Commission.

Template of Coverage Notice:

For the issuance of Notice of Workmen's Compensation Coverage Secured, all carriers are required to use the notice template prescribed and authorized by the Commissioner for such purpose (see below). ​Carriers may visit the Commission's Office to obtain a hardcopy of the notices for their usage, or request the prescribed fillable template of such notice by emailing compliance@aswcc-gov.net​​​​​​​​​​​​​​​​.​
